The surgery is presently registering new patients. We accept patients living in Hinchley Wood, Claygate,  Esher, The Dittons and some parts of Hook.

Should you wish to become a registered patient at Lantern Surgery, please complete an NHS registration form and  the health history form. These are available on-line or at the Reception. Complete these for yourself and all family members that wish to register. 

You can either print the registration form and your health history forms you need by following the links below or you can now 'PRE-REGISTER ON-LINE'

 

If you prefer to print the form please complete by hand in full and return the forms to the surgery - in person.

 

Entitlement to NHS care: 

  • You are entitled to NHS care providing you can demonstrate that you have been resident in the UK for a continuous period of 6 months prior to registration.
  • Entitlement to NHS care is not based on the payment of National Insurance Contributions or Tax.
  • Past residence in the UK (eg even as a British subject with a British Passport) does not entitle you to NHS care if you now live for 6 months or more per year anywhere overseas.

If you have not been resident in the UK for 6 months we cannot treat you through the NHS except in cases clearly needing 'immediately necessary treatment' to prevent more serious problems to your health in the future.
